The fulmer is a variation of the full cheek. The extended cheek bars, above and below the mouthpiece help to guide the horses head in the turns whilst the loose ring makes it a more mobile bit. So this bit may be useful if the horse goes better in a loose ring or leans in a fixed cheek bit.
